Gastric Emptying in Individuals with NIDDM: Relationship to Urinary Albumin Excretion

ABSTRACT
In order to assess the frequency of delayed gastric motility disturbance in non-insulin-dependent diabetes mellitus (NIDDM) and determine whe1her a relationship between the urinary albumin excretion rate and gastric motility disturbance exists, we checked the gastric emptying of a radiolabelled semisolid test meal and urinary albumin excretion in 50 patients with NIDDM. Gastric emptying of a semisolid test meal was abnormal in 22 (44%) of the NIDDM patients studied. No relationships between gastric emptying and various parameters such as age, duration of diabetes, body mass index and glycemic control were found. in contrast, the patients with delayed gastric emptying had greater urinary albumin excretion rates than those with normal gastric emptying (p<0.05). Nine patients had overt albuminuria (18%). Six of these (66.7%) had delayed gastric emptying (p<0.05). Fourteen patients (28%) had microalbuminuria. Correlation between the presence of delayed gastric emptying and the presence of microalbuminuria was noted (p<0.05). in conclusion, patients with NIDDM who have delayed gastric emptying have an increased rate of urinary albumin excretion. These two consequences of diabetes mellitus in NIDDM appear to occur together. Individuals with one should be assessed for the other. These conditions require both recognition and treatment.
